What is home care?

Services that are provided in the home for patients who have had recent surgery, are ill, or having difficulty dealing with a medical problem due to physical and/or mental limitations.

Why should I have home care?

In most cases, being able to remain at home is preferred, and less expensive. Some benefits of home care include: 

    The ability to remain in your home and independent as long as possible
    A cost saving alternative to institutional living
    Shorter and less hospital stays
    Timely care and follow-up therapy

What do you treat?

We provide monitoring, management, and instruction for all major diseases, illnesses, and post-op follow-up for adults and seniors.  Some examples of specific services are fall safety prevention, Cardiac and Pulmonary monitoring, diabetic monitoring and education, post stroke, pain management, medication management and instruction, infusion therapy, wound care, mental disorders, incontinence, and many more.

What services are provided?

The following services are provided in your home:

    Professional Skilled Nursing Services
    Physical, Occupational, and Speech Therapies
    Medical Social Services
    Home Health Aid for Personal Care

What will a Nurse do for us?

A licensed registered nurse will provide a full physical and home assessmentand report findings to your physician. The assessment includes a fall risk assessment, cardiac/pulmonary assessment, and patient/caregiver instruction and education. The nurse with the guidance of your physician will develop a care plan specifically designed for you or your loved one. 

What can therapy do for me?

Physical therapy improve your ability to move and actively participate in life’s activities by focusing on improving mobility, strengthening programs, range of motion, balance and pain management.

Occupational Therapy specializes in helping you to regain your ability to care for yourself in your activities of daily living, such as bathing, dressing, feeding, and grooming.

Speech Therapyspecializes in the treatment of speech and language disorders, as well as swallowing disorders.

What can the other services do for me?

Medical Social Worker provides assistance in resolving social or emotional problems, which may interfere with the treatment of your medical condition or recovery. They may provide information/referrals to community resources for assistance.

Home Health Aides provide assistance with personal hygiene and Activities of Daily Living such as: bathing, dressing, changing bed linens, meal preparation, etc., during their visit, depending on skills your insurance will cover.

Would I Qualify for care?

Anyone with the following qualifications may have home health services:

    Physician's order.
    Your insurance covers the services.
    You are unable to leave your home unassisted, experience a considerable and taxing effort to leave, and leave infrequently.

What will it cost me?

    A Caring Hand is Medicare participating agency and accepts Medicare Assignment. There is no cost for Medicare Beneficiary.
     A Caring Hand also participates with several private insurances.
